Incident Overview
Rig move to WEB platform faced sea condition challenges, causing vessel mispositioning.
2
Wire Entanglement & Propeller Issue
- Transfer to MV 212 resulted in wire wrapping around Aether’s propeller, creating six loops.
- Approximately 105 meters of wire with an 85-ton shackle was involved in the incident.
- Operations delayed until divers could cut the entangled lines from both sides.
3
Diver Findings & Riser Impact
- Later dives revealed three spools of wire around the 14" riser flange.
- Slack wire at 18m depth and vessel maneuvers likely caused the entanglement.
- Unconfirmed note of a possible 4" line shift, with no underwater visual evidence.
4
Corrective Actions Taken
- Additional marker buoys installed to prevent future entanglement risks.
- Wire cutting and release operations conducted to free the riser and vessel.
- Enhanced protocols for wire handling and positioning during adverse weather.
5
Lessons Learned
- Critical need for precise wire handling and vessel positioning in rough seas.
- Weather constraints significantly impact diver availability and emergency response.
- Proactive measures like marker buoys can mitigate similar incidents.
